Adjusted model 1 includes age, body mass index, black race, smoking status, diabetes mellitus, hypertension, education level, low- and high-density lipoprotein cholesterol, and physical activity; Adjusted model 2 includes model 1 covariates plus FEV1, chronic bronchitis, emphysema, and use of glucocorticoid or β-adrenergic medicines.
